How to Register

Registrations are taken year-round on a space-available basis. Please call us to find out if there is space available on your preferred days and times in the progam(s) appropriate for your children and for pricing information.

 

To reserve a space, please download, fill out and mail in the fill-able Pre-Enrollment Form and Agreement along with your non-refundable deposit. (Please download the form and open with Adobe Reader to enable the Digital Signature field. Some browsers don't allow for Digital Signatures.)

Enrollment Forms

Please download, fill out, and print the following forms for enrollment. All forms are due prior to your child's first day at Happy Days, except the Physician's Report. You have 30 days from the first date of attendance to submit the Physician's Report, so long as you can provide a copy of current immunizations on or prior to your child's first day.
